I've actually never used an excuse to leave early. I simply wouldn't go. I've used diarrhea twice with different managers (you can't use that excuse twice with the same person... it just doesn't work.).
My boss has actually let me go home for panic attacks, depression and just general "I need to go home." I think she's learned that it's better to let me go home when I'm struggling than to risk me being there and looking upset to the general public.
